Problem: Only a small number of low powered studies have assessed the relationship between mean platelet volume (MPV) and embolic disease of endocarditis. This work seeks to assess this association in the largest cohort to date and become the first to assess MPV in injection drug use associated endocarditis cases (IDU-IE). Methods: Patient charts from 1/1/2004 - 9/31/2015 were manually reviewed. Only those who reported IDU in the 3 months prior to admission were classified as IDU - IE . IDU - IE cases were stratified into those that had injected oxymorphone (OE) and those who had not injected oxymorphone (OU) . Results: MPV is significantly associated with embolic disease of endocarditis even when demographics, clinical disease characteristics, and laboratory data are controlled for (OR 1.33 [ 1.07 - 1.64 ], p=0.002). Larger MPV at admission is associated with drug use ( p=0.0004 ) . Oxymorphone injection drug use was significantly more likely to be associated with MPV greater than or equal to the median of the reference range . ( 80.2% of the OE group vs 59.5% of the OU group, p= 0.048 ) . There is no evidence that dr ug use interacts with or moderates the relationship of MPV with endocarditis embolic disease. Conclusion: MPV is significantly associated with embolic disease of endocarditis independent of vegetation size and location.en_US
